Topamax too Neurotin

Question:

My nuro is switching me from topamax to Gabapentin(Neurontin). I had the good side effect of loosing weight, but had too many bad effect. including breast pain, confusion and others. But the confusion and depression were the worse. I see Neurontin seems to be for the same problems, has any one used neurontin? and quit topamax cold? Janice Wallace Portland, Or

Do NOT quit Topamax cold! It has to be tapered off. Abruptly stopping it could bring on a seizure. Ask your doctor how to taper it off – I”m certain he won’t just stop the Topamax and start the Neurontin. But if he does, eeeeeeek! It better be a tiny dose of Topamax if he tells you to quit it cold. Topamax, Neurontin, and Depakote are 3 widely used anti-seizure drugs used to prevent migraines, and I’ve taken them all at one time or another. They tried Neurontin on me in the migraine hospital, and for a few weeks afterwards, until I started seeing my living room as a different shape with the furniture all stacked up weird! Neurontin seemed to be keeping my migraines better behaved, but I couldn’t take the whattayacallems – "reality distortions." :D But that’s ME… not everybody reacts the same way to the same drug, and a lot of people here are taking Neurontin. Let them chime in on how well it’s working for them. Do try it, because your other choice in that class of drugs – Depakote – tends to put weight on. I took Depakote for at least the last 6 years, and put on at least 20 to 25 pounds from the damn stuff. It worked fairly well for my migraines for maybe 5 years, then petered out, and I quit it 6 months ago, since it didn’t seem to be helping anymore. Then I switched to Topamax and lost 10 pounds, but had to cut back the dose on it, too, because of side effects. I’ve tried Neurontin for a few months, had absolutely no effect, good or bad, on me. I agree with Ginnie…don’t do it cold turkey.  My doctor had me taper off the same way I went on.  I kept a journal, so I knew the dosages and how long it took me.  For me it was like walking back the way I came, but I noticed it quite a bit and I wasn’t even on a heavy dosage. Michelle

EEk – My neuro told me to only quit Topamax COLD if (and a BIG IF at that) I start showing signs of glaucoma, but to also call his office right away if that happened Erika Take out the trash to email me

I’ve tried Neurontin. Had no effect on my migraine. The bad side effects I had were poor short term memory and poor concentration, but they were tolerable. I did not observe any other side effects. I’ve decided against trying Topamax due to its weight loss effect. My weight’s rather low to start with, and I am concerned of losing any more. Jay

Changing over from Topomax to Neurontin is different from stopping anti-seizure drugs altogether.  A doctor who is familiar with both drugs might have you switch directly from Topomax to an "equivalent dose" of Neurontin.  This avoids an extended change-over period when you would be inadequately medicated, first tapering down the Topomax to nothing, then gradually tapering up the Neurontin.  That’s what they do for people who have out-of-control seizures, so there’s not a gap in coverage.   A more cautious approach, when there’s more fear of side effects than of leaving the patient under-medicated, would be to taper part of the way down, and then change over.  Maybe Janice would gradually reduce her Topomax to half what she’s taking now (that would probably diminish the side effects.)  Then she could switch over directly to a low dose of Neurontin.  If she tolerates the Neurontin well, and needs more migraine control, she could increase the dose then.  Or decrease it, if Neurontin gives her bad side effects.  Discuss it with the neurologist in advance, and don’t be afraid to call the neuro’s office with questions during the transition. Neurontin didn’t help me at all (neither did Depakote.)  Topomax, Depakote, and Neurontin are all anti-seizure meds used for migraine prevention, but different ones will be more effective for different people.  I’m afraid you need to try for yourself to know what will work best for you. I would warn against starting any of the tricyclic anti-depressants, especially when you are making the changeover.  They tend to make seizures more likely, in people who are susceptible to them.  It’s a side effect that not all doctors seem to be aware of, and they certainly don’t connect it with the increased risk of seizure that comes with quitting certain migraine drugs.  I’ve had worlds of trouble with the "reality distortions," as Ginnie described, and absence seizures.
