Timoptic XE
Question:
My wife has been taking Timoptic XE drops once a day for several months, apparently without side effects and with fair results on IOP. It seems to be the most effective, after trying other drops for several years. Laser surgery may be the next step, since inital IOP of 19 seems to be creeping up to 23 while on Timoptic XE. Field test is OK so far. I do not see this medication mentioned in these postings. Is there any data or comparisons with other drops available ? Thanks and Happy New Year ! Eric K.

Hello Eric, Timoptic XE is the long acting version of Timoptic (timolol) which is the longest serving of the beta blocker type of eye drops. The once a day formulation is relatively new compared with the original, but achieves the same level of control. As with any beta blocker type drop, there can be difficult side effects if a particular patient is susceptible to this class of drugs, however, the vast majority of patients on beta blocker drops find them effective with relatively few side effects. Indeed one of the advantaged of the XE formulation is that by using it only once daily, the risk of systemic side effects is reduced. Compliance (using the drug regularly and on time) is more important with XE than the twice daily drop because the overlap of effect (the safety margin) is somewhat shorter. As far as comparisons with other drops goes, there is data available for almost all the other medications comparing them with Timoptic (timolol) in its twice daily form and this drug tends to be considered the ‘gold standard’ when establishing the effectiveness of the newer drugs. Most are about as effective as timolol, some slightly more, some slightly less and each has its own range of side effects. In the case of different classes of drugs, such as the carbonic anhydrase inhibitors (Trusopt), alpha 2 agonists (Alphagan) and the prostaglandins (Xalatan) each has a different mode of action and they can be added together to produce a greater effect. There is some evidence that switching from one drug to another as the first becomes less effective may as effective as adding another drug to the management regime for some patients. There is are also a few combination drops available where two medications are provided in one bottle, Timpilo is one example which combines timolol and pilocarpine and Cosopt is another which combines Trusopt and Timoptic in one drop, but neither can offer once a day dosing. The only other non beta blocking drops which offers once a day is Xalatan. Have been using this product for four years upon refilling today my Pharmacist advised me that once the container is opened the contents should only be used for thirty days. one container has lasted me three months, using one drop a day along with cosopt.Are other users of this product aware of this. It is the first time i have been advised of this.

My specialist told me that it would be alright to use the drops within 6 weeks.

I was not able to tolerate Tim optic XE. Hair loss, breathing problems, tightness in the chest However, Alphagan combined with Xalatan and Dr. Richard Wilson( wills-glauoma.org. in Philadelphia Pa. are working just great. It took me years to find this help.. .Of all the different laser surgeries I had, none was effective. Plus, do not ever ever let someone use THE HOLMIUM LASER, NEVER!!! I have allso been on Timoptic XE drops for some time. I have no problems . Previously i did suffer effects from other drops I used Good Luck Giellie
