The main cause for optic nerve cupping other than glaucoma is simply inheriting a nerve with a large cup to begin with. Many myopes (nearsighted people) have large eyes with large nerves that also happen to have large cups in the center of them. These generally do not get progressively larger with time. Such patients are often called glaucoma suspects and are followed with serial visual field tests and photos of the optic nerve or computerized optic nerve head analysis (HRT, GDx, etc.). If the field test and nerve stay stable over time, you have nothing to worry about. Rick Cohn, MD Glaucoma Specialist Winter Park, FL
